How to complete Form G-1145

- Complete the G-1145 form. It’s just five fields. You’ll need to enter your first name, middle name, last name, email address, and mobile phone number. That’s it. If your email address or phone number changes after you mail your package, contact USCIS directly to update your information.
- Submit the form with your application. Once completed, submit Form G-1145 along with your immigration petition to USCIS. It’s typically filed as the first page of your application package.

Once your package arrives at the USCIS lockbox, your Form G-1145 is scanned first. Within 24 hours after USCIS accepts your application, you should receive an email or text confirming receipt.
